Definition of Personally Identifiable Information (PII)
PII is information that can be used to distinguish or trace an individual’s identity, alone, or when combined with other personal or identifying information that is linked or linkable to a specific individual. Examples of PII include: name, social security number, address, email address, and date of birth.
Collection of Information
An insurance agent needs to collect and use PII in the context of assisting with giving quotes, the completion of applications for enrollment in health insurance, assistance with claims, assistance with billing issues, and other related duties. In doing so, an agent may only collect, use, or disclose PII as necessary to carry out these functions.
We may collect this information directly from you orally or on applications or other forms. This information may include, but is not limited to: your name, age, address, social security number, telephone number, occupation, income, tax status, and other demographic information about you and your family; your past, present or future physical, mental or behavioral health or condition; your health care history; your prescription information; your history of insurance coverage, premiums, claims and payments.
Safeguards to Protect Your Personal Information
We recognize the need to prevent unauthorized access to the information we collect, including that held in an electronic format on our computer systems or on hard copy. We maintain physical, electronic and procedural safeguards intended to protect all non-public, personal information.
Internal Access to Information
Access to personal, nonpublic information is limited to those people who need the information to provide our customers with products or services. These people are expected to protect this information from inappropriate access, disclosure and modification. Disciplinary action will be taken on anyone who does not abide by our privacy standards.
